Australia’s Health Tracker is a series of resources that help to provide a comprehensive assessment of the health of Australians in relation to chronic diseases and their risk factors. The resources, including this website and national report cards, help to track progress towards targets for a healthier Australia by 2025. To enquire about commissioning a report card for your PHN, Council or State, email AHPC here.

A national collaboration of public health and chronic disease organisations and experts produced health targets to support, guide and track progress towards a substantial change in the health of our nation. This work was informed by the World Health Organization’s global agenda on non-communicable diseases for realisation by 2025. See more here.
Data in the website are presented in maps and reported on by Population Health Area, Local Government Area, Primary Health Network, and at the State and Territory level. The local area profiles can be used to inform service planning, commissioning and research and can also be used to compare the data from one area against the health targets. You can also access the notes and definitions; and once you open the atlas, you can view the Help file to guide you.
